Dr. Crane participates in-network with some insurance panels. Please note that you are responsible for calling your insurance company to understand your mental health benefits and will be responsible for the full fee of any service provided that is not covered by your insurance. You are responsible for paying any deductible, co-payment or co-insurance directly to Dr. Crane. You are also responsible for obtaining any required pre-authorization for services and providing Dr. Crane with any necessary paperwork.
There may be other companies that are not listed above that cover Dr. Crane’s services or offer out-of-network benefits. Patients with other carriers must pay Dr. Crane’s full fee at the time of service. If requested, she will provide a statement that can be submitted directly to the insurance company for out-of-network benefits.
Prior to scheduling an appointment, call your insurance carrier to:
Verify that Dr. Crane is covered by your insurance for an outpatient, mental health office visit.
Obtain information about your co-payment or any co-insurance.
Obtain information about your deductible to determine if the deductible has been met for the current year.
Obtain any authorization number for outpatient mental health services.
